What is actually wrong with it

Start by naming the symptom precisely, because the four common ones have nothing in common mechanically. A tank that never stops refilling, a handle that does nothing, water appearing at the floor and a flush that needs two goes are four separate repairs.

Ask which part was tested before the fixture was blamed. On a running toilet that means the dye test; on a leak at the base it means whether the flange was looked at. A quote to reset a toilet that does not mention the flange has not established why the last seal failed.
